Peace PreSchool Registration Form 

To enroll fill out the registration form and return it completed, along with a non-refundable registration fee of $50 per child. Make checks payable to Peace Lutheran Church. Please contact Barbara Bradtmueller at 260.456.6605 if you have questions or need more information.

Drop-off or mail completed forms to Peace Lutheran Church, Attn: Barb Bradtmueller, 4900 Fairfield Ave. Fort Wayne, IN 46807. Registration forms are also available in the Church or Preschool offices.
